On my system Kmod2.8 will not run in D3D but runs fine in OGL (this is odd because most other games are just the opposite, running fine in D3d). The error of following kind:
W_CacheLumpNum: 19816 >= numlumps
usually means you have a wrong WAD file, which is missing some necessary data.

Using Hexen 1.1 patch with the default Kmod2.8 install, and runing the wad file through that gblsp(?) utility. FWIW, I can seldom play more than about 45 minutes without a crash and that "numlump" message. But, I save frequently and Hexen restarts quickly so I've learned to live with it (and it's seldom I have time for more than 30-40minutes of Bishop-bashing anyway).

FTR - recently acquired the Death Kings wad. Have played through three times, now on fourth game. It has not given me that "numlump" crash even once, so it must be something with the default hexen.wad. One odd thing I have noticed: the crash frequently, not always, occurs when there are a number of those screeching bats on screen, and whenever I kill one there is a brief image of the fighter's fist striking (just the gauntlet, no arm graphics - and I usually play as the cleric, occasionally as the mage) as I hear the "death sound" for the bat.
